PAYMENT POLICY

In all cases, Maryam Ghasemyeh, DDS patients agree to the following payment policies:

1.   Payment in full of the estimated patient portion of the fees is due at the time of treatment.

2.   Maryam Ghasemyeh, DDS does not require any pre-payment for comprehensive treatment plans requiring multiple office visits. The amount of treatment for the visit is due before care. For example, Stage 1 treatment is paid before receiving Stage 1 care. Stage 2 treatment is paid before receiving Stage 2 care.

3.   Maryam Ghasemyeh, DDS patients who have completed their stage 1 dental care and would like to pre-pay for their phase stay two treatments will receive a 5% discount on the total payment amount. To receive this discount, patients must make an initial non-refundable deposit before they return home from their stage 1 dental care.

4.   at their discretion, patients may pay in full, in advance, for comprehensive treatment plans and receive a 5% discount on the total amount of their treatment.

REFUND POLICY

You may discontinue treatment and request a refund from Maryam Ghasemyeh, DDS, anytime. Maryam Ghasemyeh, DDS will refund any amount paid for treatment you did not receive. All refunds will be processed back to the original form of payment, except cash payments, which will be refunded by cash. Written notice for refund must be submitted at least 48 hours before the expected price.

TREATMENT CANCELLATION

Patients requiring crown or bridge services may cancel treatment at no charge before the natural teeth are prepared or altered for the prosthetic restoration. When treatment has been initiated, and tooth preparation occurs, patients are liable for the estimated total cost of the services regardless of if they choose not to complete treatment.
